Attached are my final forms for the forms that you need to fill out.
The secretary, Marilyn, with the lawyer's office said that everything would
be the same as mine, except for your personal stuff.
Email your forms to: marilyn@aitkenlaw.com this weekend.
I will be emailing all of mine to her and will be sending the hard copies along
with the reproduction of the documents that they are
requesting. Jeff, you are a defendant also - look at Nos. 10 and
11 - you may have medical bills and drug prescriptions that you have
because of the incident.
